Start at the very beginning again.
This may sound silly, but it will work.
Start with grooming and getting comfortable with the horse.
Do all your lessons at a walk. Don't work too hard at it, just enjoy the movement. Don't let ANYONE else tell you when you are ready to move faster. If you are really frightened--have someone walk you on a lead or lunge line. Keep this up and you will get your confidence back. I had a very serious fall a few years back and was even unable to walk....I learn to walk again by crawling....which seems silly, but the doctors were not helping and I tried to figure out how to learn to walk again....I had five children, and they all crawled first...then walked. This actually worked for me. I think it had to do with build strength again. I was frightened when I rode for two or three months, but now I am fine....I love riding and don't even think about the possibility of a fall now. I have learned to enjoy every second on the backs of my horses. Don't let anyone else push you....you are the only one who will know when enough is enough.
